Through searches in the laboratory databases we identify 16 isolates of A. urinae causing bacteraemia during a 6-year period in southern Sweden Conclusions: Aerococcus urinae is a rare microorganism usually found in the urinary tract, which until recently was thought to be a urinary contaminant lacking clinical significance. It is now being recognized as an emerging cause of UTIs, especially in elderly males with urologic conditions, with the potential to cause bacteremia if not appropriately treated. Aerococcus urinae, which has been more extensively studied, also infects elderly men with urinary tract abnormalities, but patients with A urinae bacteremia seem to have less neurological disease [7]. / Clinical and microbiological features of bacteraemia with Aerococcus urinae. I: Clinical Microbiology and Infection.
We described the Aerococcus urinae is a Gram-positive bacterium which can cause urinary tract infections (UTIs), bacteremia, and infective endocarditis (IE) [1–3]. After the introduction of MALDI-TOF MS for species determination in medical microbiology, A. urinae has been increasingly identified as a relatively common cause of both UTIs and bacteremia [ 4 , 5 ]. Of these, Aerococcus urinae was isolated in 10 (50%), Aerococcus viridans in 6 (30%), and Aerococcus species (not speciated) in 4 (20%) patients. Seven of 20 (35%) had blood cultures with polymicrobial growth, 3 with coagulase-negative staphylococci, 2 with Enterococcus faecalis and the other 3 with Acinetobacter baumannii, Diphtheroids, and Proteus mirabilis [ Table 1 ]. 2020-04-23 · Melnick S, Nazir S, Hingorani R, Wexler P. Aerococcus urinae, a rare cause of infective endocarditis.
A. urinae is often misclassified in routine diagnostic laboratories. Cases of endocarditis (n = 7) were high-grade, monomicrobial bacteremia caused by Aerococcus urinae.
Aerococcus urinae is found in the urinary tract and most, but not all, studies implicate A. urinae as a pathogen in urinary tract infections [ 1,2,4,7,8 ]. Importantly, A. urinae has been described as a cause of invasive infections such as infectious endocarditis (IE) [ 9 ], spondylodiscitis [ 10 ], septicaemia [ 11 ], cellulitis [ 7 ] and peritonitis [ 12 ].
Isolates of this genus were originally isolated in 1953 from samples collected in the air and dust of occupied rooms and were distinguished by their tetrad cellular arrangements. Preliminary urinalysis showed leukocyte esterase, white blood cells, and many bacteria, with the culture growing mixed flora. Blood cultures revealed gram positive cocci in clusters (2/4 bottles), which ultimately speciated after additional testing as Aerococcus urinae.

After the introduction of MALDI-TOF MS for species determination in medical microbiology, A. urinae has been increasingly identified as a relatively common cause of both UTIs and bacteremia [4, 5].
A. urinae levels of 0.3 to 0.8% in urine specimens are reported, with a preponderance in patients with underlying urologic conditions ( 6 , 8 ). Conclusions: Aerococcus urinae is a rare microorganism usually found in the urinary tract, which until recently was thought to be a urinary contaminant lacking clinical significance. It is now being recognized as an emerging cause of UTIs, especially in elderly males with urologic conditions, with the potential to cause bacteremia if not appropriately treated. We present a case of an 80 year-old male with bladder cancer and multimorbidity who developed A. urinae infection. A. urinae may cause simple and complicated UTIs, bacteremia, and endocarditis in older adults with multimorbidity, chronic urinary retention, or indwelling catheters.
Through searches in the laboratory databases we identify 16 iso- Aerococcus urinae (A.urinae) is a rare bacterium that can occur in a urinary tract infection, infective endocarditis, blood stream, and spinal disc infections. A. urinae is a gram-positive coccus occurring in tetrads or pairs, similar to the α-hemolytic Streptococci, which is commonly found in the urinary tract in children and elderly people.
